Two large horizontal black coal marks at left temple. Large irregular scar in center forehead. Scar on bridge of nose; nose has been broken. Scar 1 in. long 1 in. above right brow. Scar on right cheek 1 in. from outer corner of mouth. Vertical crease between eyes. Back of head covered with small scars.
